Ok - did come investigation - there is also a tag which I had forgotten about - <$DXInlineCommentLink$> - this provides a link in a comment list to the main content document - with all the comments listed in the document.

I had again though forgotten to include the anchor in the link - I also discovered a bug in that the anchors were being placed at the end of the comment instead of the beginning.

Anyway this has now all been fixed and is in version 3 - with any luck I can this this template posted up at the end of today.
